_Cardiopulmonary Rehabilitation at the Medical Mall – Smithfield_
_Our Cardiopulmonary Rehab Department is dedicated to helping patients recover and improve their heart and lung health through structured exercise, education, and personalized care plans. Located in the Medical Mall in Smithfield, we provide a supportive environment where patients can safely work toward better health following a cardiac event or pulmonary diagnosis._
_Our program runs three days a week, with structured exercise sessions tailored to each patient's needs. During these sessions, our team monitors patients, observes exercise tolerance, tracks vital signs, and ensures safe progression through the program. On the other two days, we conduct one-on-one orientations, helping new patients understand the program and setting individualized 0-30-60-90 day care plans to guide their recovery._
_Throughout the program, our staff provide ongoing education, encouragement, and progress tracking, ensuring each patient receives comprehensive support. With a Monday–Friday schedule and closures on weekends and holidays, our team remains focused on delivering high-quality, patient-centered care in a structured and encouraging setting._
Summary:
Develops and implements an individualized care plan for patients in cardiac rehabilitation.
Responsibilities:
1. Maintains an environment of safety for patients, self and others.
2. Documents patient's status, according to unit specific procedure.
3. Develops an individualized plan of care utilizing the nursing and medical diagnosis. Collaborates with other health care team members in formulating plan.
4. Implements the nursing plan of care on assigned patients.
5. Evaluates patient's status, nursing instructions, appropriateness of the nursing plan of care, and outcomes of the care provided.
6. Initiates a patient education plan according to the individualized needs of the patient, as prescribed by Physician and/or hospital procedure, including patient and family instruction as well as discharge planning.

**Education Requirements:**
● Bachelor's degree (BSN) from an accredited school of Nursing preferred.
**Licensure/Certification Requirements:**
● Must have NC RN Licensure.
● RN NC LIC;BCLS
**Professional Experience Requirements:**
● Minimum one year experience in cardiac rehab nursing.
**Knowledge/Skills/and Abilities Requirements:**
● Language Skills: Ability to read, analyze, and interpret clinical information and technical procedures. Ability to document clinical information. Ability to effectively communicate information and respond to questions from patients, physicians, family members, and other Rex staff. Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions.
