Under general supervision, provides patient education, support, interventions, care coordination, health promotion and prevention to various cardiac populations. Will use a process of enrollment, assessment, planning, and care in accordance with physician orders, the established plan of care, the patient’s diagnosis, policies and procedures, and the Nurse Practice. The nurse will take a health coach approach to engage with and support patients in shared decision of behavior modification, achieving their goals and improving quality of life using the Mount Lasting Change Model, Motivational Interviewing, Fogg Model, and the Transtheoretical Model of Behavior Change. Associate's degree in Nursing and currently licensed as an RN in the State of Alabama required. Must have Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) through either AHA or American Red Cross. Critical care or home health experience is preferred.
