Home Care RN Job Summary




The RN Case Manager is responsible for the clinical management of an assigned patient population.
The RN Case Manager develops a patient centered, comprehensive plan of care utilizing a multidisciplinary team approach. 
The RN Case Manager is accountable for achieving positive patient outcomes
Supervises Home Health Aides and LPN’s according to policy
Reports daily to the Clinical Manager all pertinent patient information
Reports/records information accurately
Uses appropriate safety and infection control measures




Home Care RN Qualifications




Graduate of an accredited school of nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact.
Minimum of one (1) year of acute care or home care experience required. Evidence of current CPR certification required.
Valid North Carolina Driver’s License and proof of auto liability insurance required.
