The Hartford HealthCare Center for Healthy Aging is a resource and assessment center designed to make it easier for seniors, their loved ones and caregivers to access essential information and services to attain the optimal quality of life. The Resource Coordinator/Veteran Liaison is responsible for supporting veterans throughout the Hartford HealthCare system across all of Connecticut. Responsibilities include assessing psychosocial needs of clients and connecting them to community-based resources including veterans benefits. This role will help bridge the gaps in services as people transition across the health care continuum. * Bachelor’s degree in Health & Human Services or related field * Ability to travel statewide on a regular basis * Three years’ experience working with Veterans * Strong written and verbal communication skills – ability to present to large groups * Ability to navigate the Hartford Healthcare System and coordinate with community and government agencies * Organized, with the ability to manage multiple priorities * Compassionate, empathetic relationship building ability We take great care of careers.
