Essential Functions
• Participates in the maintenance of resident records including review of documentation performed by team
members. Record reviews include care and service documentation, monthly MAR reviews, setting up new
resident, and thinning records appropriately. Ensures resident records are updated and maintained per policy and state regulation throughout the residents’ stay.
• Effectively communicates with residents, family, and team members on an as needed basis to meet resident needs.
• Responsible for assisting in the screening, onboarding, and ongoing training of the care team as directed by the Director of Health & Wellness. This includes ensuring that care staff maintain appropriate annual training in alignment with Company training plans and state requirements.
• Responsible for overseeing direct care staff as directed by the Director of Health & Wellness. This may include supporting the Director of Health & Wellness in monitoring performance, coaching to improve skills, directing staff to provide activities that correspond to care plans and appropriate documentation and compliance with all human resource guidelines.
• Maintains and communicates resident bathing, housekeeping, laundry, and other resident service schedules.
• Supports maintenance of a safe Memory Support environment.
• Verifies all associate hours on timecards and weekly hours report.
• Coordinates monthly staff schedules, arranges replacement staffing when necessary and maintains clear
records of all changes, requires and irregularities. Maintaining the care team schedule and on-call plan to
assure proper coverage to meet resident needs and according to budget. This includes system data entry and
maintenance.
• Monitors resident call light response by team members and reports findings to the Executive Director or
Director of Health & Wellness.
• Assigns physician ordered daily, weekly, and monthly resident care services.
• Monitors staff in the administration of medication to ensure order compliance, within scope ,completes audits of
medication administration record.
• Assists the Director Health and Wellness in medication administration management, including administration
of medications to residents, ordering medication from pharmacy, and maintaining records as dictated in
company policies and procedures manual. Assures that medications are properly stored, medication
carts/cupboards are cleaned, and medication room is organized according to standards.
• Reviews capabilities of residents who self-administer medications on a regular basis as required by Elegance
and state requirements.
• Observes residents’ health statuses, takes actions to address concerns within scope of practice, and reports all
significant changes, reactions to medications and treatment, or significant incidences to the Director of Health
and Wellness in a timely manner.
• Communicates with families regarding residents’ personal needs and achievements. Reminds families of
need for scheduled physician, dental, or eye appointments and supply needs.
• Assists in organizing and developing service plan reviews as required in coordination with appropriate
resident care team members and resident families.
• Maintains Service Plan Binders and Service Plan Check-off lists or other documentation as appropriate.
Monitors documentation daily to ensure completion.
• Responds promptly to all emergency calls from residents residing in the community (IL, AL, PC, and MS) as
directed.
• Performs daily, weekly, and monthly safety and infection control checks of the home and grounds, assisting
the Executive Director and the Director of Health and Wellness for resident health and safety purposes.
• Assures in-house ancillary services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc.
are scheduled and followed through.
• Ensures that resident transportation to medical appointments is coordinated and completed.
• Shares on call duties as required within scope.
• Participates in community functions including family socials and open houses. Communicates with
community professionals in a professional and appropriate manner.
• Responsible for supply inventory control, ordering, distribution, and organization with budget.
• Responsible for understanding and supporting the Director of Health & Wellness in implementing local,
state, and federal rules and regulations as they apply the delivery of care and service in the community. This
may include but not be limited to team member training and oversight, , and compliance with HIPAA, OSHA,
Blood Borne Pathogen Guidelines, and other rules that apply to the work we do.
• Ensures the community team works in a safe manner and leads correction unsafe actions.
• Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions which assist
others beyond job responsibilities, and has a positive impact on co-workers, residents, and visitors.
• Ensures residents are treated with respect and dignity, recognizing individual needs, and encouraging
independence. Fosters a homelike atmosphere throughout the community.
