No County Queens Street Address Hillside Horizons Day Habilitation Program 197-27 Hillside Ave. City Holliswood State NY Zip Code 11423 Duties Description Independently performs the duties and activities of a Rehabilitation Assistant 1 listed below; and may supervise and train Rehabilitation Assistants 1 and/or direct care staff, and perform the full range of supervisory duties.Provides paraprofessional rehabilitation services to individuals in psychosocial support, vocational rehabilitation, competency training, and residential services program areas.• Interviews and gathers individuals’ information; assists in conducting assessments to determine individuals’ potential and existing capabilities and interests; and in developing and implementing rehabilitation plans, including goals and objectives.• Assists in planning and conducting group rehabilitation activities, and in developing group protocols and curriculum.• Observes and reports individuals’ participation, response, and behavior; and may recommend modifications to rehabilitation plans based on observations.• Participates in treatment team meetings; and assists in determining individuals’ progress and rehabilitation needs.• Maintains records and documentation in accordance with program standards and requirements.• May monitor and maintain supply inventories; and schedule and arrange for rehabilitation activities and special events.•Planning, organizing, and supervision of the implementation of Day Hab. programs for all individuals who attend Day Hab.•Participation in Individuals’ Life Plan meetings and any special meetings needed. •Development of Staff Action Plans based on each individual’s valued outcomes as discussed during Life Plan meetings. •Review of Life Plans for accuracy•Ensure completion of due diligence to get finalized Life Plans for all individuals served.•Ensure submission of Staff Action Plans to Care Managers in the required time frames. •Development of classroom schedules and lesson plans to ensure Valued Outcomes are carried out as per the Life Plan and Staff Action Plans. •Ensure billing documentation is accurate and completed daily in preparation for billing.•Completion of timely Monthly Notes •Ensure EHR is being utilized to upload monthly documentation, Staff Action Plans and Life plans. •Ensures each individuals’ record contains current documentation (Life Plan, LOC/LCED, Staff Action Plan, Daily Service Recordings, Monthly notes, Behavior Support Plans/Documentation, Safeguard Summary)•As needed, visits assigned IRAs when Day Habilitation Services are being provided from the residences. •Attends and participates in all Mandatory Trainings, in-services and staff meetings.•Additional responsibilities as needed, assigned at the discretion of the Program Coordinator, or Treatment Team Leader. Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.• Please note that starting salary for this title reflects the downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 14 ($52,198) and Downstate Adjustment ($3,400).
