requires travel to our home base hubs out of Hartford and New Britain then travel in company fleet to clinic sitesJob DescriptionThe Registered Nurse RN will provide professional nursing care to patients following established standards and practices as a member of the Neighborhood Health team Promote protect and preserve the health of the public through promoting healthy lifestyle prevent disease and health problems provide direct care educate community about managing chronic conditions and making healthy choicesAssess and evaluate community health needs including future needs plan appropriate interventions for identified health needs of the community provide comprehensive nursing services on health promotionillness prevention and counseling These activities contribute to meeting the health care safety and comfort needs of patients and are performed in accordance with all existing regulatory state and company standards policies and proceduresWork where every moment
