In addition, this role is a remote position

The remote employee will report to a designated MHS work site in the following circumstances:

Mandatory education. Staff will be notified in advance in order to make the necessary arrangements.

Equipment failure: The supervisor must be contacted immediately. In the event the equipment cannot be repaired quickly; the remote employee will be required to report to a designated MHS work site for the remainder of their assigned shift and any shifts thereafter until the equipment is repaired.

Mandatory meetings.

MultiCare Health System will not reimburse for travel and mileage when reporting to a designated MHS work site.

ESSENTIAL RESPONSIBILITIES

1. Demonstrates working knowledge of various program guidelines, admission requirements, and regulatory standards for all home care services including Home Health, Hospice, and Palliative Care.

2. Responsible for concurrent and retrospective audit/review and tracking of clinical case data/records for the purpose of supporting and improving achievement of optimal clinical, financial and core measure benchmark outcomes and complying with all applicable standards.

3. Ensures accurate and timely completion of regulatory and payor-specific orders, plans of care, discharges/transfers, wound care order entries, and quality item sets (OASIS/HIS).

4. Reviews reports, monitors alerts, and trends data within benchmarking database (currently SHP).

5. Mentors and/or coordinates education of clinical staff in quality assurance findings and best practices regarding quality documentation and/or clinical processes.

6. Maintains current knowledge and understanding of diagnosis coding and payment impact.

7. Demonstrates working knowledge of disease specific protocols/order sets to ensure consistent standards of practice, resolving discrepancies as indicated.

8. Identifies potential or actual areas of concern, interprets actual trends and develops reports displaying variances from established regulatory and care standards.

9. Participates in webinars, teleconferences and educational opportunities to remain current in quality measure regulatory requirements.

10. Triages patients/cases based on medical records, referral communication and patient interaction.

11. Communicates constructively with providers to ensure collaboration/coordination of care.

12. Communicates frequently with support staff, field staff and leadership.

13. Maintains competency on all computers systems/software utilized for completion of role responsibilities.

QUALIFICATIONS AND REQUIREMENTS

Experience: One year of Hospice, Home Health, or Palliative Care experience required.

Licenses/Certificates/Registration: Valid driver’s license and proof of automobile insurance required.

Additional Requirements: Cell phone for use during working hours; car for travel to and from home visits during shift.


FTE: 1.0, Shift: Days, Schedule: M-F 8-4:30pm, Rotating weekends and call

Requirements

Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreementAdvanced degree in Nursing (MSN, PhD, DNP, ARNP) preferredRegistered Nurse license (RN) in Washington State or multi-state licensure endorsement (MSL)Current BLS Certification requiredACLS or NALS or PALS or TNCC or NRP certification, as required by facility or departmentCertification in specialty area preferredOne (1) year of experience in acute care setting preferred

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $41.94 - $75.89 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
