Comunilife is looking for a Nurse to join their Medical Respite Program in Harlem. In collaboration with the Program team, the Registered Nurse (RN) conducts mental and physical health assessments, plans, develops, coordinates and provides treatment and rehabilitation, as well as professional support, and makes referrals to community resources, including community physicians and coordinates consultations from other health professionals. The RN is responsible for the management and administration of medication within their scope of practice and in consultation with the attending physician. Responsibilities and Duties
Responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and supportive services. Provides comprehensive primary health care, including but not limited to, case finding, health screening, health promotion, physical and psychosocial assessment, case management, and overall management of medications. Ensure and maintain principles, and procedures that are in agreement with a Nursing licensure, Program guidelines, and the Department of Mental Health and Office of Mental Health regulations. Develop, maintain, and revise written plans of care and general health care records. Engage new clients in the continuum of care through outreach. Collaborate with other health care providers and make appropriate referrals. Serve as client advocate specifically in accessing the health care system. Provide communication to team members and participate in community and apartment visits to clients. Coordinate services with other community mental health services such as, inpatient, outpatient, emergency, or day treatment. Assist in the development of a program that closes gaps in the continuum of services. Affirm that all record keeping activities, including but not limited to, databases, daily logs, progress notes, and client charts are appropriately maintained and current. Assist in the training of team members in the range of health promotion and illness prevention, treatment, rehabilitation, and support services available. Serve as a health care resource to clients and extended families. In both self and group processes. Assist in the preparation of any and all clinical correspondence relative to the activities of theTeam as requested by the City, State, or Federal funding sources.
