ERP is an Equal Opportunity Employer - Disability and Veteran. **Responsibilities** **Work Schedule:** Monday - Friday, 8.5 - 9 hours between 7:00am and 7:00pm, to include a 30 - 60 min uncompensated lunch break No Weekends or Holidays No Overtime **Job Specific Details and Tasks:** Duties may include the following, dependent upon department: • Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness. • Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF. • Perform physical exam and health histories. • Provide health promotions, counseling, and education. • Administer medications, wound care, and numerous other personalized interventions. • Direct and supervise care provided by other healthcare professionals. • Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel. • Recognize adverse signs and symptoms and quickly react in emergency situations. • Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. • Make referral appointments and arrange specialty care as appropriate. • Conduct research in support of improved practice and patient outcomes. • Attend staff meetings to provide case discussion, continuing education, and continuous quality improvement and safety. • Ability to effectively utilize an electronic health record and other software applications, or revert to paper documentation if systems are unavailable, to appropriately document assessments, interventions, and evaluation of patients’ plans of care.  Maintain a level of productivity comparable with that of other individuals performing similar services.  Participate in peer review and performance improvement activities.  Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.  Function with an awareness and application of safety procedures.  Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, leadership, or other designated person regarding problems that the HCW is unable to manage.  Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.  Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting.  Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.  Maintain an awareness of responsibility and accountability for own professional practice.  Participate in continuing education to meet own professional growth.  Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.  Participate in the provision of in-service training to clinic/ward/unit staff members. Assist with providing training and/or clinical direction as applicable to supporting government HCWs (e.g., hospital corpsmen, students, etc.).  Attend and participate in various meetings as directed.  Perform timely, accurate, and concise documentation of patient care.  Operate and manipulate automated systems such as ADS, MHS Genesis participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications.  Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions.  Assess each patient and perform triage duties as assigned.  Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes, and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies.  Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes.  Provide treatment and discharge instructions upon patient release.  Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status.  Provide a safe and clean environment for each patient.  Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.  Assist in planning, provide clinical direction and provide instruction to LPNs/LVNs, nursing assistants and ancillary personnel.  Follow up with patient referred by Primary Care Manager (PCM) or Internal, Behavior Health Consultant (IBHC) and focus on patient progress in PCM treatment adherence.  As required, participate in PCM or IBHC case staffing and consultations that involve patients actively engaged in or may benefit in care facilitator services.  Participate in screening processes developed by the IBHC and PCM to improve the recognition of and appropriate interventions for depression and PTSD.  As required, provide, and teach curriculum content for classes that promote education and skill-building to enhance enrollee population psychological and physical health.  As part of the MHP team, the HCWs shall raise awareness about behavioral health issues and care facilitator services in MHP practice area to promote easy referral of patients.  Inform the Clinic Manager of administrative/clinical problems as they arise.  Participate in all accreditation and Medical Home Port recognition processes (JC, IG, etc.).  Engage other MHP team members to assist in improving documentation of the episode of care using MHP Clinic-approved templates as the initial template (i.e., Tri- Service Workflow/COMPASS).  Coordinate schedule and non-availability (i.e., leave, training, etc.) with other BHIPMHP team members to ensure adequate access to care.  Participate in MHP team huddles. **Qualifications** **Minimum Qualifications:** *** Education:** Minimum ADN from an accredited college or university *** Experience:** Possess a minimum of 1 year outpatient experience as an ambulatory care nurse within the last 2 years, with experience providing services in psychiatric nursing, behavioral health and/or wellness counseling. *** Licensure:** Current, full, active, and unrestricted RN license from any state. *** Life Support Certifications:** AHA or ARC BLS Certification *** Security:** Must possess ability to pass a Government background check/security clearance. _The salary range for this position is $39.20/hr. - $76/hr.
