The individual will work with the patient, family, significant other and the interdisciplinary team members to develop and individualized plan for care which includes educational and discharge planning interventions. The Registered Nurse Hospital Critical Care and the interdisciplinary team members will evaluate the plan of care and will address unmet outcomes to determine further intervention or consultation(s) as necessary. **JOB QUALIFICATIONS** **EDUCATION** _For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation._ **Minimum Required:** Diploma or associate or bachelor’s degree in nursing. **Preferred/Optional:** None **EXPERIENCE** **Minimum Required:** Graduated from an accredited nursing school. Will need to have passed nursing boards prior to start date. **Preferred/Optional:** Experience as a registered nurse in an acute care setting. (Medical Intensive Care Unit (MICU) & Surgical Intensive Care Unity (SICU): One year of experience as a registered nurse in an intensive care unit. **CERTIFICATIONS/LICENSES** _The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position._ **Minimum Required:** Current State of Wisconsin Registered Nurse license or Nurse Compact License. Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications awarded by the American Heart Association within three months of hire. **Preferred/Optional:** Trauma Nurse Core Course (TNCC) awarded by Emergency Nurses Association.
