Position Purpose:
To provide individualized nursing care at the place of residence/home of clients with acute, chronic and complex conditions.
What you will Do:
Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis.
Provides health coaching/education to patients/families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER/Urgent usage.
Delivers nursing care for the clients in their homes, utilizing appropriate available resources.
Involves the client/caregiver in developing and carrying out the plan of care.
Minimum Qualifications:
Education:
Current state Registered Nurse License or permit from State Board of Nursing required.
Completion of ADN required, BSN preferred. Experience: Must have demonstrated competency in an acute patient care setting for at least one year in medical, surgical or intensive care setting. Home Health, community health, or hospice experience preferred.
