SUMMARY
The RN Visit Nurse provides nursing care to patients in collaboration with assigned case managers.
Homecare: Ensure the health and safety of all Home Care clients is provided through development, implementation and evaluation of individual client care plans.
Hospice: Provide end of life services to patient with a terminal diagnosis needing hospice services in their home or community setting, assisting case manager in the development, implementation and evaluation of care plans.
PRIMARY RESPONSIBILITIES
Provide direct care to residence under individual plans of care, administering care and medications per doctor’s orders.Act as an advocate to maintain client privacy and dignity during cares. Administer prescribed medications and treatments per doctor’s orders.Monitor resident vitals, completing resident assessments as assigned. Communicate any identified changes to physician and family members.Document patient problems, nursing assessments, patient goals, care provided, and client/family outcomes from interventions and care provided.Collaborate with the IDT (interdisciplinary team) and Hospice IDG (interdisciplinary group) as needed to ensure adequate and appropriate flow of services.Periodically perform the comprehensive assessment of residents with acute changes of condition, and attend resident Care Conferences as needed.Assist throughout the admission process, to ensure smooth transition of new residents.Assure clients maintain admission criteria; reassess and facilitate alternative placement if necessary.Supervise, oversee and lead unlicensed staff as needed, ensuring Minnesota Home Care Rule and Statute requirements are met.Serve as a clinical resource to staff regarding care of residents. Provide in-service education as requested.Maintain and complete all required documentation and charting on the computer or on forms in an accurate and timely manner according to policies and procedures. Accurately transcribe Physician’s Orders as per facility protocol and forward necessary information to other departments.Support various other Hospice and Home Care responsibilities as needed.
JOB SPECIFICATIONS
Education, Experience and Credentials
Must be licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.CPR/First Aid certified required.Two or more years’ previous related experience in a similar position and in a similar care environment preferred.Knowledge of Medicare regulations in home care/hospice, OASIS certification and palliative care knowledge preferred.Possess a valid driver’s license with good driving record, and proof of valid insurance. Possess effective problem solving skills.Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must be skilled in database management and record keeping.Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.Able to understand and follow facility and department safety and emergency procedures.
PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB
Incumbent must be able to:
Perform heavy work, lifting up to 50 pounds, and frequently lifting/carrying of objects weighing up to 20 pounds as needed.Effectively communicate in the English language, including following oral and written instructions, in order to communicate with people inside and outside the organization.Able to effectively use office equipment to review and create information.Work at a desk for extended periods and perform long hours of work at a computer. Frequently stand and walk to move around office/buildings repeatedly throughout the day.Able to work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.Able to obtain reliable transportation.
TOOLS AND EQUIPMENT USED
Use thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment.Use telephone, computer and standard office equipment.
WORK ENVIRONMENT
The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to respond to the needs of residents, in responding to emergencies. May be at risk of exposure to blood, bodily fluids, and other potentially infectious material.
