Referral Coordinator (Non-Exempt)
This is a hybrid work from home position. Overview: To support seamless patient transition into agency's services by interfacing with hospital/facility staff, physicians, and families to coordinate and process patient referrals. Qualifications: Education: Must have high school diploma or equivalent with successfully completed course work. Experience: Must have one (1) year of experience working in a healthcare environment. Clerical and computer skills required, including data entry, office machines, and working knowledge of phone systems. Must be proficient in the use of Microsoft programs for internal communications (email) and database tracking/reporting, as well as proficiency working with electronic health records.
