With a dual focus on both nurturing client relationships and ensuring a smooth, continuous flow of referral communication and follow-up. + Develop and maintain relationships with the existing referral sources, such as hospitals, medical professionals, therapists, and community organizations for the purpose of streamlining patient care. + Responsible for ensuring that coordination of care between clients and referral sources are maintained as deemed appropriate by client. + Act as the point of contact for referrers as it pertains to patient care. + Coordinate and conduct regular check-ins with referral sources, for patient updates, addressing any concerns and gathering feedback. + Maintain strong rapport with counselors and clientele. + Follow up with client's post treatment to gather feedback, testimonials, and success stories and to ensure successful transition into their next level of care. + Coordinate with the facility's care and administrative teams to implement suggested improvements when applicable. + Collaborate with the marketing team to share, develop, and implement strategies and improved systems monthly. + Maintain accurate and updated records of all client and referral source interactions in the facility's EMR ad CRM system. + Attend discharge meetings to coordinate with referral sources. + Assist with discharge planning and client discharge plan completion as needed. + Performs other duties as assigned by the Clinical Director. Requirements + Bachelors Degree preferred. + Preferred Qualified Health Professional (QHP); CASAC, LMSW or LMHC-P in NYS. + Experience in client relations, case management, marketing, or outreach, preferably within a healthcare or substance facility setting is preferred. Salary Description $50k - $55k
