Recreational Coordinator/Case Manager

POSITION SUMMARY
Reports to: Program Director
Trainings: Annual, QPR, HSFAC
To provide a wide range of recreational activities appropriate to the needs and interests of program participants as well as to students in the District, in an effort to promote health, fitness and well being.
Recreation Coordinator
Duties and Responsibilities:
• Organize and administer a series of regularly scheduled, ongoing activities utilizing school gymnasiums, tennis courts, fields, and other recreational facilities after school hours, on weekends and during summer hours.
• Responsible as lead chaperone on all trips unless other arrangements have been made prior to the date of trip/activity.
• Devise and establish special programs to meet the needs of such groups as the physically handicapped and/or educationally handicapped.
• Develop a monthly calendar with planned activities and events, and distribute to SBYSP, Teachers, Counselors and other Support Staff with the District.
• Administer and coordinate schedules and activities with school calendar and activities in mind, in an effort to avoid conflicts. (i.e. School play, talent show, etc).
• Work with Program Director in selecting and purchasing equipment and supplies.
• Responsible for all trip and school permission paperwork and approvals (i.e. Field Trip Request forms, Facilities Use forms, etc).
• Work with other SBYS Staff as to referrals and information on procedures.
• All other duties as assigned.
Counselor Social Services:
Duties and Responsibilities:
• Assists in the completion of screening form
• Accurately reports information to team members and participates with team members and clients/families in the completion of a Service Plan within established time frames.
• Demonstrates a knowledge of crisis intervention and behavior management of aggressive behavior.
• Assists in the implementation of psychoeducational programs.
• Demonstrates a knowledge of community resources.
• Accurately documents all client interaction within established time frames.
• Writes in a professional and competent manner.
• Maintains a valid NJ driver’s license.
• Practices safe and courteous driving habits.
• Provides services to clients/families in their native language or is able to procure the services of a translator.
• Demonstrates a willingness to serve on professional committees, attend staff meetings, attend in-services, etc.
• Seeks supervision in a professional manner and documents supervision.
• Participates with team members and clients/families in discharge planning.
• Develops an adequate professional relationship with all referral sources and relevant community agencies.
• Adequately responds to the needs of clients, families and referral sources.
• Provides core services to clients as assigned
• As appropriate to scope of practice, assists in the data collection for each assigned client, including the following:
The relationship of each clients physical status to dependence.
The nature of the comparison to use alcohol and/or other drugs.
The intensity of the clients mental preoccupation with using alcohol and/or drugs.
Qualifications:
Bachelor’s Degree in Social Sciences (Social Work/Psychology/Sociology) with Sports background preferred.
Experience working with adolescents required.
Requirements: New Jersey Valid Driver’s License
CPI & HSFAC required within 3 months of date of hire or transfer
Equipment Use: Computer, Copier, Fax Machine
