The Physical Therapist provides evaluation and sets up treatment programs for all patients referred to physical therapy; provides physical therapy treatment to referred patients; writes and reviews patients progress reports, including charting, and reports to referring physicians. Additional responsibilities include:
Documents evaluations and treatment goals and plans; provides correct information on Medicare, AHCCS and all other forms; regularly update s documentation and maintains all required prescriptions and signaturesDocuments the course of patient care including progress made and continuing need for treatment; provides correct statistics and billing information on all PT patients; provides patient and family education on tasks and resources that will assist with continued functional independenceEvaluates patient for limitations including but not limited to ROM, strength, sensation, balance, and coordination and identify functional deficits to communicate findings to referring physician. Reassesses patients as conditions change at appropriate intervals.Develops treatment plans that are problem oriented (ie., specific to those limitations outlined in the evaluation or re-assessmentEstablishes goals for the patient that are objective and measurable, and which can be divided into achievable short term and long-term goals. Includes patient/ family goals as integral part of program.Provides treatment intervention which may consist of therapeutic modalities and/or procedures, prosthetic training, orthotic fabrication/ fitting and training, training in use of assistive devices, and provision of patient/family education to provide a smooth transition toward maximum level of functional independenceMinimum Qualifications:
Education: Bachelor's degree required.
