The Psychiatric Social Worker/Program Specialist will join the Ellis Outpatient Mental Health Clinic. The mission of the clinic is to provide person-centered, holistic, recovery-oriented care so individuals can achieve their life goals. This employee will have the opportunity to provide direct care and advocacy for a diverse and vulnerable population. The clinic team specializes in a wide range of treatment interventions, including trauma-informed care and integrated mental health and substance use treatment.
The Psychiatric Social Worker/Program Specialist is responsible for assisting with the workings of the Ellis Clinic. This position is devoted to the following areas: crisis response/coordination, auditing and utilization reviews to improve care on a systemic level, NIMRS and Incident reporting and investigation, assistance with intake coordination (under the leadership and direction of the Intake Coordinator), care management services, and group therapy coordination, among other assigned tasks. The remaining time is spent as a Psychiatric Social Worker, providing thorough and clinically appropriate social work interventions, this includes individual and group psychotherapy, psychosocial assessments, diagnosing, person-centered treatment planning, crisis intervention, and family/collateral engagement.
EDUCATION AND EXPERIENCE REQUIREMENT(S):
+ Master’s Degree in Social Work, LMSW licensure required, LCSW preferred
PRIMARY RESPONSIBILITIES OF THE POSITION:
Program Specialist Responsibilities:
+ Reports to Clinical Manager of Outpatient Services and the Clinical Supervisors; carries out duties as assigned
+ Maintains accurate databases and keeps clinical notes, including utilization of Powerchart, Mental Health App, Soarian Financial, and Referral Manager
+ Assists the Intake Coordinator with all intake-related duties, as assigned, including but not limited to triaging referrals, scheduling intakes, managing databases of referrals
+ Acts as liaison to other community agencies in intake-related issues, including attending discharge meetings with inpatient, crisis, Office of Community Services, and corrections departments to coordinate high need patients as needed in collaboration with the clinical supervisor and Intake Coordinator
+ Leads the crisis response at the clinic, including responding to “backup” calls and walk-ins, in coordination with clinical staff, supervisors, and the Living Room.
+ Completes all audits (for individual and group notes) and utilization reviews to remain in compliance with OMH standards
+ Completes NIMRS/incident reports and investigations and presents findings at the PI meetings in order to improve EMHC care/systems.
+ Provides care management services for EMHC patients, as needed, in support of EMHC clinical staff
+ Coordinates group-based therapy at the Ellis clinic, including but not limited to helping clinicians establish/schedule groups, maintaining the list of groups, and advertising/outreach to patients to refer to groups
Psychiatric Social Worker II Responsibilities:
+ Provides care/coordination to a diverse population of people diagnosed with Serious and Persistent Mental Illness (SPMI)
+ Carries a caseload of up to 15 patients; the number will be calculated based on acuity and need of patients, as well clinical/supervisory judgement. The majority of patients served at the clinic are higher need/higher acuity individuals
+ The responsibility of the individual psychotherapy based on the treatment need of each individual patient includes providing evidence-based treatments such as CBT, DBT, ACT, psychodynamic psychotherapy, and other clinically appropriate interventions. Option to facilitate clinical groups, including trauma and substance use-related groups under the supervision of those track leaders.
+ Advocates for an underserved population through coordination with primary care, care management, and other community partnerships.
+ Assess determinants of health and make referrals for appropriate services, including housing, transportation, medical providers, and care management.
+ Attends individual supervision (frequency determined by supervisor), bimonthly group supervision, and weekly team meetings with the MD/NPP staff.
+ Facilitates and/or attends patient-centered meetings with community providers required for optimal patient care
+ Provides intake assessments on new patients over the course of 1-3 sessions and makes formal diagnoses and recommendations that will guide further treatment at the clinic
+ Utilizes the CASE approach to suicide/homicide risk to assess safety and provide accurate interventions to mitigate harm to patient and community
+ Submits all documentation within 72 business hours and ensure yearly treatment plans are up to date with each billable service
+ Coordinates treatment with nursing and MDs/NPPs to ensure high quality patient care
+ Provides clinical services to patients who are on Assisted Outpatient Treatment or other court-mandated treatment through the County and/or judicial system; this includes writing the AOT orders for the treatment team, submitting them to the county, and coordinating/attending all AOT meetings

Salary Range: $31.50-47.26 /hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
