**Job Summary**
The Department of Neurology is looking for a Nurse Practitioner for a position in the Headache Division. This position provides comprehensive evaluation and treatment for patients with headache and facial pain syndromes. This is an outpatient position.
Current RQI Healthcare Provider eCredentialed through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
**Responsibilities**
Essential Functions
Takes complex medical histories on patients referred for consultation by academic and community physicians.
Performs general and full neurologic examinations
Formulates the diagnosis and treatment plan
Consults with physician(s) regarding patient health care plans and treatment.
Diagnose and develop treatment plans for complex headache conditions (if needed in collaboration with headache division chief and other members ad hoc)
Orders, interprets, and evaluates diagnostic tests to identify and assess the patient’s clinical problems and health care needs.
Competency with procedures associated with headache to include and not limited to (Botox for chronic migraine, Trigger Point injections and Occipital Nerve blocks) This will require observation and credentialing after hire.
Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy.
Complies with medical records policies regarding the complete and accurate documentation of patient care. Initiates and maintains required records and legal documents.
Participates in quality improvement activities.
Counsels patients, families and significant others about health and illness, and promotes health maintenance.
Problem Solving
Ability to perform the essential functions of the job as outlined above.
Demonstrated clinical competence and in-depth knowledge of specialty area.
Demonstrated leadership, teaching, research and communication skills.
Ability to provide care appropriate to the population served.
**Minimum Qualifications**
Required
Licensure to practice as a Nurse Practitioner in the State of Utah.
Preferred
Certification in ACLS / BLS / NRP / ATLS may be required by your department or by privileging criteria.
Utah State prescriptive privileges.
Licensure is obtained by completing a Master’s degree in Nursing or equivalency (the only equivalency to the Master’s degree is current licensure as a Nurse Practitioner) and a National Certification which requires the successful completion of a nationally accredited examination.
UHPPM 1-56 requires that newly hired non-physician providers apply for credentialing and privileging at the time of hire.
This position is patient-sensitive and must fulfill all associated requirements.
