We are seeking a ESLP Program Manager to join our team in Escondido, CA
The ESLP Program Manager oversees the provision of person-centered, community based support for individuals with disabilities. The manager takes active ownership of assigned caseload by managing and coordinating cases from initial assessment through housing placement. This role has the rewarding opportunity to support our individuals in their daily lives and help them achieve full inclusion within their community. In addition, they’re responsible for providing direct supervision to the Lead and Direct Support Professionals, budgeting, developing and maintaining individual service plans and ensuring quality care standards are met.
Job Duties:
Oversee day to day operations of program and acts as liaison to day program, job sites and adjunct services for the individuals.
Conducts low income housing search activities leading to individuals successful placement in the community that is suited to their individualized needs, choices and desires.
Assists in the development of goals/objectives for support and acts as a member of the planning team.
Schedule and attend all required individual related meetings.
Respond to calls/text messages/emails from individuals or members of the individuals planning team within 24 hours or the next business day if it falls on a weekend. Respond promptly to emergency calls regarding individuals or staff shift call outs during non-traditional working hours.
Ensure that individual goals and behavior plans are implemented and documented as written.
As needed, provides direct living services to ensure program individuals have the support and supervision necessary.
Responsible for hiring and develops training and assistance to all Lead and Direct Support Professionals.
Follow up on personnel and meeting attendance tracking, disciplinary actions, performance improvement plans as necessary.
Assist individuals with management of funds including requests, expenditures, receipts, reconciliation and record keeping.
Assists and support individuals choices and interests by identifying and implementing preferred activities.
Coordinates staff schedules with assigned Lead Direct Support Professional.
Ensure the availability of all prescribed medications’ remain alert to behavioral or medical changes in any residents; monitor for staff adherence to medication administration protocols for individuals; notify appropriate personnel regarding side effects or adverse reactions as needed.
Actively observes for signs of neglect, maltreatment, or violation of rights, and takes immediate action to remedy situation and support advocacy in this process, reporting internally and to outside agencies as required by law.
Maintains confidential and accurate individual files and records.
May perform other related duties when assigned or required.
Salary: $75,000 - $80,000
