Constructive Partnerships Unlimited (CPU) seeks an experienced Program Manager with Community Engagement experience to conduct community outreach and engagement activities in New York City. The Program Manager will play a critical role in ensuring that Constructive Partnerships Unlimited remains connected to the communities we serve and is responsive to its needs. The Program Manager will work closely with the Social Care Network team and SCN ecosystem partners (including Medicaid populations) to help coordinate community outreach events, listening sessions, and engagement strategies to ensure meaningful and culturally relevant integration of our SCN at the hyperlocal level.

Constructive Partnerships Unlimited is a non-profit organization with locations throughout the five boroughs, Suffix, Putnam, and Dutchess counties serving individuals in various capacities. CPU is partnering with lead entities on this initiative to address the social care needs of Medicaid-eligible individuals. CPU will partner with community-based organizations and other entities to link individuals to services that they identified.
POSITION DUTIES:
+ Provide social services resource navigation expertise and support to the team, promoting high-quality care coordination, efficiency, and effectiveness.
+ Oversee the day-to-day operation of the program by ensuring that referrals are seen within 3 days, documentation of notes within 24 hours, completing monthly invoicing, and submitting to Lead Entities promptly.
+ Collaborate with internal teams and network partners to determine capacity and needs within the network.
+ Evaluate and optimize workflows ensuring seamless integration across the network.
+ Support the development and implementation of a robust framework for onboarding network partners.
+ Initiate, maintain, and deepen relationships with community partners.
+ Plan and deliver training on social service offerings and best practices, as needed.
+ Lead and conduct audits of workflow and documentation activities ensuring adherence to business standards, partner and government requirements.
+ Lead discussions with internal teams and external partners on service eligibility, and community issues as they arise.
+ Work collaboratively with Lead Entities to manage, analyze, and report on data and performance.
+ Develop and implement policies and procedures to ensure the effectiveness of program operation.
+ Provide ongoing staff training.
+ Support the development of our Healthcare-Community Partnerships, identifying new partnership opportunities and leading/supporting proposal developments, as needed.
+ Manage programmatic relationships with referral platforms and other technology vendors, troubleshooting issues and managing enhancements ensuring underlying network technology solutions facilitate network efficiency and effectiveness.
+ Ensure documentation, billing, and invoicing meet Lead Entity requirements.
+ Participate in network partner engagement meetings, staff/team meetings, mentoring meetings, planning meetings, and others, as requested.
+ Collaborate closely with the supervisor and SCN management to support the team in developing/revising screening and navigation workflows and process improvements that increase network effectiveness.
+ Identify and prepare participant success stories to demonstrate SCN's impact and promote the network.
+ Provide support for team training and productivity reporting, upon request.
+ Other assigned duties.
+ POSITION REQUIREMENTS:
+ Bachelor's degree from a four-year college or equivalent.
+ At least 3 years of supervisory experience in social service or human services environment.
+ Past direct social services case management experience is required.
+ A strong familiarity with and understanding of the public health, social services, and healthcare landscape in New York City. •
+ Initiative-taking, and highly organized with the ability to work autonomously. • Excellent presentation, communication, and people skills.
+ Proven experience leading cross-functional teams to successful outcomes.
+ Detail-oriented possessing strong analytical and problem-solving abilities.
+ Proficiency in Microsoft Office Suite tools (e.g., Word, Excel, PowerPoint).
+ Experience with data collection, reporting, and analysis. •
+ Excellent team player with the ability to be flexible, work collaboratively and respectfully both internally and externally and engage in consensus-based decision-making.
