It is a time of growth at Constructive Partnerships (CP) Unlimited! The Agency recently expanded its constituency to include homeless services.
Reporting to the Executive Director, the Program Director will provide onsite comprehensive day-to-day leadership and oversight for the ESD Program, which integrates the Critical Time Intervention (CTI) and Transitional Residential Services (TRS) components with a diverse range of duties, including, but not limited to staff supervision, compliance, program integrity, operational, clinical, and administrative duties. Some specific duties and responsibilities include:
Leading, managing and evaluating the day-to-day operations of multiple locations, including, but not limited to hospital referral sites and transitional residence in locations in Yonkers and the Hudson Valley area.
Overseeing the implementation of the CTI phased model, ensuring precision in the care transitions from hospital settings to the community.
Ensuring that the TRS component adheres to trauma-informed care and person-centered approaches, fostering an environment of safety and emotional regulation for residents.
Serving as the liaison with external stakeholders, including hospitals, OPWDD service providers, and regulatory agencies, building strategic partnerships.
Leading and supporting quality improvement initiatives, monitoring program performance, monitoring client outcomes, and compliance with state and federal regulations.
Fostering a culture of collaboration, accountability, and recovery-focused care among all ESD Program staff.
Requirements:
· A minimum of 5 years’ demonstrated experience leading and managing a team in the behavioral health field, with a focus on populations that have co-occurring mental health and I/DD needs.
· A master’s degree in human resources or related field.
· Must be a Licensed Clinical Social Worker (LCSW) in NYS.
· Extensive knowledge of trauma-informed care, care management models, and state/federal regulatory standards
· Exceptional written and verbal communication skills couples with organizational skills required.
