Program Coord-Citizens  Self Directed Services  Plainview location 35hrs flex

Assists the Assistant Director with overseeing non-traditional supports and services such as self-directed services (ex: housing subsidies, partial and full self-directed service options). As the coordinator, you will assist the Assistant Director with the Fiscal Intermediary responsibilities for self-directed services, including the oversight of Broker and Community Habilitation services for the people being served.

Qualifications:

Associates degree required,  Bachelor’s degree preferred. A minimum of five (5) year’s supervisory experience and a minimum of five (5) years’ experience supporting people with ID/DD is required. Excellent written and interpersonal skills required. 

Primary duties and responsibilities include but not limited to:

FISCAL INTERMEDIARY PROGRAM COORDINATOR: Provides administrative and billing support to a person who chooses self-direction.    

The Program coordinator will assist with:

Ensuring completion of background checks for self-hired staff; Billing Medicaid or New York State for services in the budget; Reviewing and approving of payroll for self-hired staff; Tracking expenditures; and Providing monthly statements to the person. 

 SELF-DIRECTED SERVICES:

Assists in planning the development and oversight of the operations of self-directed services including program development, supervision of community habilitation staff and Brokers, and acts as a liaison assisting the Assistant Director with the Fiscal Intermediary responsibilities for self-directed services, in order to support people to access Self Directed Services through OPWDD’s service model. Facilitates the process for people to access self-direction Assists in overseeing and supervising the Startup and Support Brokers Works with the Assistant Director of Self –Direction to work with the CFO and accounting department for the operation of the individual budgets

 

Works with the Assistant Director to create and maintain policies and procedures that align with OPWDD’s regulatory requirements and best practices, as it relates to Self-Direction. Reviews paperwork/documentation for accuracy and regulatory compliance; distributes documents to the appropriate department (i.e. HR, Accounting, Payroll). Maintains training records for Start Up and Support Brokers.

HOUSING SUBSIDIES:

Assists people with obtaining housing subsidies through the completion of the housing subsidy application, including obtaining all documentation needed to support the application; submits completed applications to OPWDD for approval. Maintains records for each person, ensuring that documentation meets regulatory standards (DDP-1, ISPs, QA Check Lists, etc.). Visits each person in their home annually in the lease year, and completes a QA Check Lists to monitor health and safety. Assists people with obtaining reimbursement for related items purchased out of pocket; when appropriate, utilizes Paramount Purchasing System and agency vendors when making purchases on behalf of the person. Completes budget renewal applications on a yearly basis; submits to OPWDD for approval. Develops and maintains relationships with landlords, management companies, and facilitates the process of lease renewal. Monitors spending of transition stipend monies. Completes reimbursement paperwork for billing of start-up funds and transition stipend; submits to OPWDD for processing.

OTHER RESPONSIBILITIES:

Reports to the Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect. Additionally, must follow all agency incident policy and procedures. Adheres to Federal and New York State regulations, agency policy and procedures, the employee handbook, and the agency and program training requirements. Performs all other related duties as requested by supervisor.
