Job Summary:
Join our dedicated team as a Primary Support Professional, where you will play a vital role in providing Individualized Supported Living (ISL) services to clients in need. In this position, you will collaborate closely with the Program Coordinator to ensure the effective implementation of Individual Support Plans (ISPs) and support clients in their daily activities. Your commitment to enhancing the quality of life for individuals with intellectual and developmental disabilities will make a significant impact in their lives and the community.

As a Primary Support Professional, you will work both in the home and community settings, ensuring that clients receive the personalized support they need. Your responsibilities will include training staff, overseeing daily activities, and maintaining a safe and healthy environment for clients.

Key Responsibilities:

Work in the home and/or community with assigned client(s) as designated by the Program Coordinator and/or the Regional Director.Participate in Individual Support Plan meetings and RN visits or service monitoring visits when possible.Train staff on each client’s medical needs, personal preferences, and compliance.Oversee daily residential notes, goals, and health tracking.Maintain house calendar and update any appointments.Ensure all annual medical appointments are up to date and report outcomes to the supervisor.Review the physical condition of the home to ensure health, safety, and cleanliness standards are met.Ensure safety kits are present and complete.Schedule staff or cover shifts for personnel who are sick or absent.Collaborate with the Program Coordinator on follow-up to monthly RN visits and trainings.

Education, Experience, and/or Credential Qualifications:

Must be 18 years of age; must have a high school diploma or GED.One year of experience in the field of intellectual/developmental disabilities or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office.

Additional Qualifications:

Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.Current driver’s license, acceptable driving record, and current auto insurance.

Physical Requirements:

ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time.
