Practice Transformation Coordinator Senior
Job Summary Develops, implements, and evaluates Population Health Management strategies and tactics across the ambulatory environment. The Program Coordinator II works to drive improvement in health outcomes, quality of care, experience of care, as well as increased engagement of providers and thereby realize enhanced value of care for the population of patients served by the organization. Operationalize and manage care transformation at the practice level by leveraging data monitoring and analysis, relationship building with practice staff and Providers, and change management strategies. Provide education and coaching on recognitions requirements; mentor, support, and provide continuous education to physicians and practice staff on the value-based reimbursement programs. Assist in presenting learning collaborative sessions. Assist in the development of learning collaborative presentations and resources. Assess practices in the adoption on the medical home model. Essential Functions + Manages assigned projects and initiatives including; health plan/payer reporting needs, develops project plans, and implements steps needed to achieve goals and timelines for pay-for-performance and quality initiatives for BCBSM (PGIP), Medicare (MSSP), Priority Health, HAP, and other health plans; assesses current state, analyze practices and provide support for required changes. + Provides support for Corewell Health network providers (primary care & specialist) to maintain compliance with value-based program requirements. Stays abreast of program changes and implements them as needed. + Establishes and maintains strong partnerships with Corewell Health ACO physicians, provider office staff, and other key stakeholders to ensure timely and accurate communication of program requirements and updates. + Collaborates with ACO team, Medical Directors, and operational leaders to ensure the development and implementation of strategies for success related quality and process improvement, utilization, and cost. + Manages multiple ambulatory practices in implementing action plans, monitors and evaluates progress as needed to achieve targets for programs. + Create and lead the education provided to practices regarding the Corewell Health ACO clinical practice improvement initiatives as well as ACO’s engagement and quality incentive programs. + Responsible for training staff regarding HEDIS measures, quality improvements principles and standards, conducting PDSA, Cycle Time analysis, Process Mapping, Gap analysis, and facilitate quality improvement. + Lead liaison between the team and ACO data staff to develop reporting procedures that will guide the management of processes and to improve compliance with specified metrics needed to meet program goals. + Responsible for ensuring practice engagement via sharing performance data and assessing current practice processes; mutually identifying areas of opportunity. + Effectively collaborates with peers within the organization to provide coordinated care and customer service to Corewell Health ACO practices. Ensures tight alignment to meet the needs of the practices. + Incorporating front-line experiences, provides recommendations to Director for improving reporting tactics and processes in order to more effectively support the Corewell Health ACO population health and quality initiatives. + Accountable for managing process improvement projects to address opportunities found in analytic interpretations and monitors the outcomes. + Responsible for leading all training with new staff members (program coordinators I) to the team to support to their development and knowledge of the BCBSM PCMH, BCN PRP programs, and other assigned duties. + Engages providers to build on their practice infrastructure which promote the concepts of patient centered care. + Performs other duties as assigned. + Travel to other office/practice sites and meeting and training locations.
