Population Health Coordinator - Part Time

Overview
With support and guidance from supervisors, coordinates internal and/or external health partnerships and project teams to plan and implement population health initiatives. This role will support the work of the statewide Maternal Health Task Force and coordinate and facilitate community perinatal coalitions. The coordinator will support the implementation of improvement projects emerging from the statewide strategic plan and the local coalition priorities. The role will ideally be filled by persons with BA degrees and experience in public health, coalition development, and co-design practices. This is a hybrid role that will support the Rochester/Dover area regional coalition. Role will be a Dartmouth Health employee but housed atHope on Haven Hillin Rochester. There will be time spent working remote, in the designated community/region, and some travel for in-person work with team.

Responsibilities
1. Support development and facilitation of community perinatal coalitions
2. Assist community perinatal coalitions to engage persons with lived perinatal expertise as co-leaders
3. Provide project management and support to community perinatal coalitions for improvement initiatives
4. Support other Project Staff to organize Listening Sessions and Journey Mapping and Photo Voice
5. Support community coalitions to collect, assess, and make informed use of local and state data to support decision-making and project implementation.
6. Upholds standards of the D-H Code of Conduct Policy and HRSA
Qualifications
* Bachelors Degree or equivalent experience.
* Minimum of 1-2 years’ experience in related field
* Strong interpersonal and organizational skills, creativity, flexibility, diplomacy, and the ability to work as part of and contribute to a positive team culture.
* Demonstrated ability to handle multiple tasks and coordinate projects under pressure with support.
