Physician Practice Coder
Position Summary Reviews and audits patient chart documentation for the purpose of ICD and CPT coding. Performs complex physician practice patient bill coding for the purposes of obtaining reimbursement to the facility. Performs a variety of clerical medical office duties to facilitate daily practice operations, keeping close communication with the physician. Arranges for the efficient and orderly registration, pre-certification and scheduling of patients.   Minimum Qualifications Work Experience One year of experience in a medical office required.
Education Associate’s degree in Health Information Management or related field preferred.
Certifications/Licensure Certified Professional Coder (CPC) or related medical coding certification required within six (6) months of hire or placement.   Codes patient encounter. Initiates and completes patient account transaction process. Reviews and audits patient chart documentation for the purpose of ICD and CPT coding. Performs complex patient bill coding for the purposes of obtaining reimbursement to the facility. Follows coding guidelines and standards of ethical coding by scrutinizing the clinical documentation within the entire medical record to perform ICD-9-CM selection of principal diagnosis, secondary diagnosis, procedure, and CPT-4 codes where appropriate.  Abstracts and enters coded information into the computer system for data collection and billing purposes. Monitors data input to ensure clean claims at point of service. Monitors the state and federal regulatory compliance requirements regarding patient billing.
