Description:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Trinity Health. Discusses patient and hospital information only among appropriate personnel in appropriately private places. Behaves in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values and policies of Trinity Health. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management. Assumes responsibility of patient care as a first line provider to new admission to the hospital. In collaboration with the attending physician, prepares and records patient history, develops and implements a plan of care, and documents the pertinent symptoms, findings, diagnoses and care plans. Works in collaboration with attending physicians, intensivists, consultants,  fellows, medical residents, APPs, nurses, and other members of the care team to provide patient care. Coordinates and performs patient assessments, and orders follow-up care, consults, and discharge orders under the directon of attending physician. Performs technical procedures as credentialed and authorized. In accordance with unit or departmental practice, proficient with the electronic health records for documentation, assessment, and care management, performing these activities concurrently with provision of care throughout the shift. Assumes responsibility for professional growth by attending continuing education conferences, procedure and device in-services and unit educational offerings. Participates in hospital wide committees regarding services and/or process improvement. Demonstrates and actively promotes an understanding and commitment to the mission ofTrinity Oakland through performing behaviors consistent with the Trinity Health Values. Maintains a working knowledge of applicable Federal, State, and Local laws and regulations, the Trinity Health Organizational Integrity Program, including the Standards of Conduct, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behaviors.

OTHER FUNCTIONS AND RESPONSIBILITIES

Performs other duties as assigned.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

Education:  

Graduate of an accredited school of Physician Assistant/MSN program or other Masters of Science degree in a health related field including six (6) to twelve (12) months of clinical internship.

Graduate of an accredited Nurse Practitioner program with a minimum of a Masters of Science in Nursing, including six (6) to twelve (12) months of clinical internship.

Experience: 

Candidate should ideally hold at least twelve (12) months of relevant work experience as an advanced practice provider, preferably in an acute care.

Certification/Licensure

Must hold minimum licensure and in "good standing" in one of the following in the State of Michigan: Physician Assistant or Nurse Practitioner.

If Physician Assistant, must hold certification from the National Commission of Certification.

Current BCLS and Advanced Cardiac Life Support (ACLS) certification.

Must be credentialed for clinical privileges through the SJMO AHP Credentials Committee and the SJMO Board of Trustees.

REQUIRED SKILLS AND ABILITIES

Excellent interpersonal skills to effectively communicate with patients, family members and the healthcare team. Has the ability to utilize computers and work effectively with the Electronic Health Record. Ability to work independently. Demonstrates initiative and motivation. Excellent customer service orientation skills necessary in order to deal effectively with various levels of hospital personnel, outside customers and community groups.
