Modify treatment plan as conditions warrant, to maintain the safety and wellbeing of each patient/client. Consult with the physical therapist if the patient's/client's condition has changed or there is a need for re-evaluation. Document accurately and completely each episode of care in a timely fashion consistent and Rehab Services department documentation guidelines.
