Heart Share Human Services of New York has four preschools located in Brooklyn and Queens focused on maximizing the potential of every student no matter the learning disability. Through the use of Applied Behavior Analysis and TEACH along with the New York State Common Core curriculum each student is provided with an education experience geared towards their needs. The Heart Share Preschool also provides different therapies and physical education courses geared to what the students want and need. The Heart Share Preschool is also a family and community that strive to help each other out with a common goal of helping the children succeed. These are necessary steps to create the impact we are looking for which is provided them the keys to succeed on their own in the future. The HeartShare preschool operates as a 12-month program.
Responsibilities:
Provide 1:1 physical therapy services to children with special needs ages 3-5 as mandated by IEP. Participate in Annual Review Process and Transdisciplinary determination of IEP goals base on the criteria of present and future environment. Design and implement goals for individual and classroom therapy. Provide therapeutic intervention using a push in and a pullout method. Implement a variety of activities and methods in a child friendly manner to facilitate physical therapy goals during therapy session. Design, integrate and train protocols in sensory diet implementation. Prepare and implement classroom based activities weekly in conjunction with the classroom teacher. Arrange overall therapy schedule. Consult with teachers and teacher assistants regarding elicitation and generalization of physical therapy goals in the classroom to maintain consistent programming. Consult with other clinicians (ex. ST, PT, Psych., SW) to discuss and implement appropriate therapeutic intervention within session. Participate in IEP updates and annual review meetings with families. Meet with individual families, as needed. Maintain contact and provide direct instruction to families regarding their child’s physical therapy goals. Participate in Transdisciplinary team meetings and follow through with any actions that are recommended in the area of physical development. Monitor physical therapy goals as well as completing quarterly progress notes reflective of the IEP. Maintain daily attendance, individual session log in EZ Trac and data collection for pertinent IEP goals as described in the policy and procedure Manual. Plan in-service training in conjunction with the Supervisor, provide in-service training to staff, and participate in staff development activities. Plan and conduct parent education groups. Remain sensitive and responsive to cultural differences present in the agency’s service population. Evaluate feeding skills and determine appropriate therapeutic intervention and follow-up including referrals to specialists (ex. video swallow, nutritionist) Participate in peer supervision. Submit weekly therapy schedules and notify Supervisor of any changes in overall schedule
