Qualifications Education: 4 Year / Bachelors Degree, required. Graduate from an accredited PT program, required.License: Appropriate state PT license, required.

• Provision of Rehabilitative Care: Delivers care that is consistent with the physical therapy practice act in the appropriate state and adheres to APTA Code of Ethics. Completes patient evaluations and treatments independently while maintaining patient safety. Requires infrequent guidance/mentoring for complex or non-routine patients. Appropriately seeks guidance and assistance when necessary. Independently establishes / implements treatment goals and plans based on patient input and evaluation and assessment results. Reassesses outcomes and modifies goals and plans to meet changing needs of the patient. Schedules patient activities according to patient individual needs. Demonstrates technical competencies and job knowledge involving unusual or infrequent circumstances. Appropriately delegates tasks to support personnel.
 
• Documentation: Documentation is timely, legible, accurate, consistent with established procedures, and inclusive of evaluation, assessment/reassessment, goals, plan, response to interventions, and discharge planning needs as permitted by licensure.
 
• Communication: Communicates with patients, families, and visitors in a caring and empathetic way. Provides timely and accurate information to all health care team members involved in the care of the patients, both within and outside of the organization.
 
• Education: Uses appropriate education methods and customizes education processes for the patient and key learner. Acquires and maintains knowledge of physical therapy practice by identifying continuing education needs and participating in educational opportunities. Provides orientation and guidance to new rehab staff in work area as requested. Assists with student development as opportunities present based on the employee's years of experience as directed by the manager.
 
• Teamwork: Develops and maintains positive relationships with team members and coworkers, both within and outside of rehabilitation. Functions as part of an interdisciplinary treatment team. Accepts responsibility for actions; seeks, accepts and acts positively upon feedback from others. Assists in meeting needs of a changing environment. Shares responsibility for effective internal communications through a team approach. Organizes individual patient caseload in collaboration with team workload demands. Performs daily administrative/office activities to insure efficient flow of care within team (e.g. new orders, discharged patients, scheduling). Actively participates and cooperates in team functions and meetings to enhance the delivery of services and team processes. Attends monthly staff meetings.
 
• Financials: Completes charge sheets in compliance with services provided. Maintains awareness of patient insurance limitations and financial resources to ensure resources are used most appropriately to meet patient’s needs in a cost-efficient manner. Meets productivity expectations of the department.
 
• Marketing: Models and insures positive customer service skills. May participate in marketing activities. Promotes collegial relationships with physician and other professionals.
 
• Human Experience: Show courage through creating and sharing innovative ideas to improve the experience for both patients and peers. Round on patients to create meaningful connections and keep patients informed of visit details (delays/wait times). Model the experience principles through consistently engaging in Always Event behaviors and viewing feedback through the patient lens. Recognize and value the unique differences and similarities in both our team members and patients to create an inclusive environment where diversity is celebrated. Explain all processes to patients in plain language and utilize teach back to ensure understanding. Know and model the mission, vision and values, and how they relate to role-specific responsibilities. Model our people credo through a passion to care for each other, our patients and our communities.
