Responsible for the assessment, re-assessment, treatment of patients in assigned areas, with maintenance of all records pertinent to treatment. This includes the delivery of physical therapy services, participating in interviews. Evaluates patients assigned for treatment. Completes the initial evaluation and report, including setting short and long-term goals, treatment and documents patient’s report in his/her medical record. Renders treatment as determined. Re-evaluates at appropriate intervals and makes the necessary modifications. Maintains records of patients’ progress and renders written reports at established intervals. Establishes and maintains treatment schedule for assigned patients; attends conferences, meetings and program rounds. Consults with medical staff regarding recommended therapy services and attends evaluation conferences and makes rounds to discuss progress of patients and problems relating to therapy.
