Duties & Responsibilities:
Establishes positive, collaborative, and consultative relationships with partner hospitals, community network providers, population health teams and community pharmacies. Partners with community pharmacies to communicate medication adherence strategies, pharmacy partnership program performance and Healthfirst pharmacy updatesServes as the engagement liaison between Healthfirst, network providers and pharmacies to triage pharmacy benefits, member grievances or pharmacy related inquiries.Acts as a resource to provide pharmacy education, best practices, and training related to pharmacy partnership program.Performs review of medication lists to understand and eliminate barriers to medication therapy while serving as the member's Pharmacy Navigator in order to achieve the following:Proper Medication AdherenceMinimize any gaps in therapyPrevention of medication related adverse events through collaboration with a pharmacistProvides weekly status reports detailing outreach efforts to the reporting on-site manager and Healthfirst managerPartners with the pharmacists on the team and other members of the healthcare team to communicate program related IT issues and grievances Facilitates member access to medication(s) by:Contacting pharmacies to obtain medications refillsContacting members’ providers to issue new prescriptions and set up appointments when necessaryFacilitating access to medications for members experiencing financial issues.Working with the member’s pharmacy to align refills to help members reduce frequency of pharmacy visits.Documenting the member outreach activities and communicating information to other providers.May coordinate and integrate an intervention within the broader healthcare management services being provided to the patient.Participates in monthly or quarterly meetings as required, including material preparation and updates.Additional duties as necessaryMinimum Qualifications:
Meets at least one of the following:Nationally certified Pharmacy Technician in good standing (i.e., PTCB, NHA) or registered or licensed Pharmacy Technician in good standingHigh School Diploma or GED from an accredited institutionWork experience requiring written and verbal communication that is clear, concise, grammatically correct, and professionalExperience communicating with patients, providers, and pharmacists on medication and medication related issuesKnowledge of MS Office applications (Word, Excel, PPT, Outlook) Experience in health care or managed care. This can include work experience and/or education. Ability to travel throughout New York City and Long IslandPreferred Qualifications:
Experience creating and updating spreadsheets Experience with a health plan's quality improvement programCompliance & Regulatory Responsibilities: Noted above
License/Certification:
Meets at least one of the following:Nationally certified Pharmacy Technician in good standing (i.e., PTCB, NHA) or registered or licensed Pharmacy Technician in good standingHiring Range*:
Greater New York City Area (NY, NJ, CT residents): $47,403 - $62,400
All Other Locations (within approved locations): $41,101 - $60,320
As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
