Pharmacy Supervisor
Summary The Clinical Pharmacy Services Supervisor has responsibility to develop, organize, manage and control pharmaceutical care for patients within the clinical pharmacy programs and functions as a supervisory level program manager in accordance with the guidance set forth in Pharmacy Service policies and procedures, VA regulations and Federal laws governing the practice of pharmacy. Responsibilities Assures adequate provision of pharmaceutical care to established clinics/patient care areas on a daily basis in assigned work areas. Ensures subordinates document adverse drug events, clinical interventions, drug information provided and patient drug therapy evaluations and counseling in the patient's medical record and other documentation areas as required and appropriate. Ensures clinical competency of staff using tools such as Pertinence Reviews and education modules. Provides intensive medication therapy management services for chronic disease states. Functions as a mid-level provider, will initiate and modify a patient's medication therapy in order to maximize patient response and minimize drug interactions, adverse drug effects, and polypharmacy while providing cost-effective treatment. Has advanced skill and knowledge necessary to order, review and analyze appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to implement the necessary changes in the plan of therapy. Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the provider as appropriate. Actively involved in reviewing patient medication regimens for clinical effectiveness, least toxic and most economical medication treatments, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Participates in budgetary execution including but not limited to monitoring requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable, designing and assisting with therapeutic conversions. Promotes and monitors compliance for established drug therapy policies. Works with providers to ensure compliance with national, VISN, and local initiatives. Makes provisions for the development and training needs of staff. Provides initial job training and information for applicable staff. Coordinates training of new pharmacy employees and re-training of others annually regarding current policies and procedures, environmental care issues, etc., and makes assessments regarding an individual's capability to perform specified responsibilities. Serves as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians, and students. Uses appropriate references to research drug information. Provides timely and accurate responses to drug information inquiries from all customers. Evaluates prescriber's medication orders for appropriateness, renal/hepatic dosing, drug interactions, allergies/adverse reactions, and duplicate therapy. Supervises technical staff to maintain efficient and effective operations. Works with other health professionals to develop and participate in the implementation of protocols for clinical and health systems research related to pharmacy. Develops and coordinates clinical research activities and projects to benefit the Pharmacy Service. Serves as a mentor in the development and conduct of resident research projects including development of research questions, methodology, application to IRB, and assisting with presentation and manuscript preparation as appropriate. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet- Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations GS-13 Pharmacy Supervisor. In addition to the basic requirements for employment stated above, the following criteria must be met when determining the grade of candidates. Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Demonstrate the following KSAs: Ability to effectively supervise subordinate staff. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining to pharmacy. Skill in managing people or programs. Preferred Experience: Successful completion of PGY-1 Residency Program and three (3) years of clinical practice experience as a CPP in an ambulatory care setting, or successful completion of a PGY-2 Residency Program with at least once year of clinical practice experience in an ambulatory care setting.
