Members of the Clinical Pharmacy Integration team will be responsible for medication management quality, cost, utilization and clinical stewardship activities at practice and program level.
MSHP seeks a **PGY2 Ambulatory Care Pharmacy Resident** to...
PGY2 Ambulatory Care Pharmacy Resident who works under general/direct supervision and reports to the Director of Pharmacy and the Director of the PGY2 Ambulatory Care Pharmacy Residency Program, Population Health, MSHP. The PGY2 Ambulatory Care Pharmacy Residency is designed to transition the PGY1 residency graduate from general practice to a specialized pharmacy practice specific to the needs of ambulatory patients. The resident will have direct patient care responsibilities aligned with care delivery goals as dictated jointly by MSHP and practice leadership. The resident will perform all duties in accordance with the residency standards set forth by the PGY2 program and the Pharmacy Department.
The resident practices within an interdisciplinary environment and graduates with the ability to develop and secure collaborative drug therapy management agreements with providers at ambulatory practices. Resident will lead medication management performance improvement initiatives. Resident will assist with design, implementation and evaluation of all related pharmacy initiatives. Resident is expected to assist with pharmacy students and PGY1 pharmacy residents as requested.
**Qualifications**
**Education and Experience**
+ PharmD from an ACPE accredited degree program
+ Completion of an ASHP accredited PGY1 Pharmacy Residency or PGY1 Community-based Pharmacy Residency
**Certifications/licensure**
NYS pharmacist license
NYS Board of Pharmacy
+ If NYS pharmacist license is expected to be pending at the start of the residency, the resident must have an active NYS Pharmacist Limited (Intern) permit by the start of residency. Proof of applying towards license and or permit by start day in order to orientate.
**Additional Skills and Qualities**
+ Registration with National Matching Program
New York Stat pharmacy license or license eligible. Must have valid New York State intern permit if license eligible.

**Responsibilities**
**Responsibility #1 Delivery of clinical pharmacy services with a focus on chronic care management, transitions of care, and care management**
+ Promote clinically rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs
+ Serve as a clinical pharmacy resource for the health-system and its affiliated pharmacy services for affiliated providers in the Clinically Integrated Network (CIN)
**Responsibility #2 Pharmacy Strategy Innovation**
+ Identify, develop and implement new clinical pharmacy strategies, operational improvements, and innovation pilot projects across applicable clinics
+ Provide training to clinic staff and practitioners on program improvement activities, as required
**Responsibility #3 Education**
+ Provide scholarly activities and presentations at local and national meetings to advocate for and raise awareness of population health pharmacy integration and innovations
+ Provide education to staff and patients on processes such as medication reconciliation and disease state management
+ Lead and participate in pharmacy resident conference
**Responsibility #4 Quality/Utilization Management**
+ Assist with scaling MTM and chronic disease management program and managing goals of pharmacy integration across MSHS, CIN employed or voluntary practices
+ Support financial, quality and utilization analytics, performance improvement evaluations, and impact to value based care metrics
+ Serve as pharmacy representative on behalf of MSHP as required at organizational committees
**Responsibility #5 Documentation**
+ Document all activities in the electronic medical record and other databases using the established format in a timely and accurate manner
+ Communicate promptly with practitioners and other team members to relay pertinent findings
**Responsibility #6 Staffing**
+ Complete longitudinal ambulatory staffing experience every other Saturday
+ Involved with community outreach and public health initiatives

**Compensation Statement**
Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $58661 - $60000 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
