Job Summary:
The Pharmacist in the Specialty and Retail Pharmacy serves as a clinical liaison between specialty clinics, retail pharmacy operations, and patients, ensuring the delivery of high-quality patient-centered care. This role involves managing both specialty and retail pharmacy operations, focusing on medication adherence, therapeutic outcomes, and comprehensive patient support. The pharmacist will engage in direct patient care, collaborate with interdisciplinary teams, and participate in clinical initiatives to optimize outcomes for complex therapies, particularly within the 340B program framework.
Responsibilities:
Oversee accurate preparation, verification, and dispensing of medications in both retail and specialty settings, ensuring compliance with state and federal regulations Coordinate workflow across specialty and retail services to ensure seamless patient transitions and timely access to medications Manage inventory to ensure medication availability while minimizing waste and optimizing procurement under the 340B program Utilize and maintain patient management software for tracking interventions and documenting outcomes in both specialty and retail contexts Provide medication counseling and education, focusing on specialty medications, adherence strategies, and complex therapies Manage the enrollment and onboarding process for new specialty pharmacy patients, ensuring smooth coordination of care Conduct medication therapy management (MTM) and follow-up assessments, particularly for patients with chronic and complex conditions Serve as a resource for healthcare providers, patients, and caregivers, offering drug information and coverage assistance Collaborate with prescribers, nurses, and other clinical staff to develop and manage individualized care plans for specialty pharmacy patients Participate in clinic rounds, team-based care models, and quality improvement initiatives, ensuring pharmacy services are integrated into patient care workflows Coordinate prior authorization and financial assistance efforts, working with specialty pharmacy technicians to ensure timely access to therapies Ensure adherence to all regulatory requirements, including 340B program compliance, specialty pharmacy accreditation, and medication safety protocols Participate in the monitoring and reporting of key clinical and operational metrics, supporting continuous quality improvement efforts Engage in audits and compliance reviews, ensuring pharmacy operations align with hospital standards and external regulatory requirements Precept pharmacy students, residents, and interns, fostering their development in specialty and retail pharmacy practices. Participate in clinical research initiatives, publications, and presentations to contribute to the advancement of pharmacy practice.
