This adherence applies to all compliance and conformance plans, policies, codes of conduct, laws and regulations in effect. Reviews, receives/verifies all new patient orders and documents. Assists physician’s and other healthcare professionals involved in planning and implementing patient care to ensure safe and effective drug therapy as necessary. Provides consultation and answers inquired from patients, families, physicians and other healthcare professionals regarding medications and their use. Instructs/counsels patients and caregivers regarding specific drug therapy including possible adverse drug reactions and/or interactions. Identifies appropriate outcomes of drug therap. Monitors and documents ongoing drug therapy including the assessment of: Therapeutic appropriateness of the choice of drug(s) Therapeutic duplication within patient’s drug regimen. Appropriateness of dose, frequency and route of administration. Adherence to drug regimen, potential drug/food/or diagnostic test interactions; disease limitations to proper drug use. Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians and other healthcare professionals. Responsible for ordering, inventory, maintenance of medications. Prepares, labels, packages, and dispenses medication orders.
