Pay: $62.19 - $87.07 / hour. Pay is dependent on applicant's relevant experience
Minimum Requirements:
Doctor of Pharmacy from an accredited school of Pharmacy.
State licensure as a Pharmacist and any relevant life support certification as determined at position level.
5 years specialty experience OR PGY1 or clinical fellowship completion with 3 years' pertinent work experience OR Pertinent PGY2 completion.
Position Overview
This pharmacist is responsible for developing, implementing, and monitoring antimicrobial stewardship clinical activities for their designated hospital(s). They coordinate local and/or regional antimicrobial stewardship program efforts, including regulatory preparedness, tracking and monitoring antibiotic use and resistance data, and developing and implementing evidence-based guidelines to improve antimicrobial use. This will align with system priorities and translate from local to system work when applicable. They participate in departmental and interdisciplinary committees to improve the safety, therapeutic outcomes, and cost savings related to antimicrobial use. They serve as a clinical resource for providers and pharmacists and lead the development and dissemination of educational materials. All local and regional work is to be coordinated and evaluated for system opportunity. This position reports directly to the UCHealth Infectious Diseases Pharmacy Supervisor.
Job Duties
+ Utilize the EHR decision support tool to complete direct patient care activities
+ Including, but not limited to, positive blood cultures, drug-bug mismatches, prospective audit and feedback for protected antimicrobials, de-escalations, and antibiotic time-out for broad-spectrum antimicrobials
+ Design and provide effective education for providers, nurses, and pharmacists related to appropriate antimicrobial use
+ Own, develop, and oversee implementation of antimicrobial-related order-sets and agile pathways, including ongoing maintenance, education, and accountability for utilization in alignment with system antimicrobial stewardship work.
+ Identify antimicrobial-related medication use evaluation (MUE) opportunities; collect, track, and report data for MUEs and quality improvement initiatives
+ Provide leadership and ownership of projects related to quality, safety, and cost-effectiveness of antimicrobial use
+ Lead and precept infectious diseases-related research projects and share results at local, state, and/or national level
+ Precept clinical rotations for pharmacy learners (students, PGY1 residents, and/or PGY2 residents)
+ Maintain existing antimicrobial-related collaborative practice agreements, including renewal through regional pharmacy and therapeutics committees and auditing protocol adherence, as appropriate.
+ Provide pharmacist support for Infectious Diseases Consult services, as able
