Summary: Provides pharmaceutical care services, medication and equipment to the patients of Parkview Home Infusion. Services include, but are not limited to: oversight, monitoring, preparation, dispensing, and documentation of patient medication therapy. Provides advice to physicians and Infusion / Hospice staff on a variety of topics related to medications and other Pharmacy-related matters. Consults with appropriate individuals pertaining to safe and effective medication usage in the home. Ensures compliance with Federal, State, and local laws and regulations as well as compliance with regulatory agencies affecting the Pharmacy. Education: Minimum of a B. S. degree in Pharmacy from an accredited school of Pharmacy. A Pharm. D. degree is preferred. Licensure/Certification: Must be eligible for or hold a valid and current license as a Registered Pharmacist from the Indiana State Board of Pharmacy. If eligible, must obtain Indiana licensure within 180 days. Additional certifications may be required for specified areas. Must have reliable transportation and provide proof of vehicle insurance. Must have a valid driver's license. Experience: One to two years home infusion or hospital pharmacist experience preferred. Must have excellent verbal and written communication skills. Must be available to work overtime or flexible hours as needed. Must successfully pass a criminal background check. Must have good verbal and written communication skills. Must be able to lift, push, pull or handle at least 50 pounds.
