PERSONAL CARE ASSISTANT

 

Location:         Mercy LIFE West Philadelphia

Status:             Full time 

Shift:                 Monday through Friday 8 am to 430 pm

Position Purpose: 

The personal care assistant/worker is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. 

 

 

Position Details: 

This position will primarily consist of home care assignments. However, occasional facility work will also be required. 

Position will require working one weekend a month and rotating holiday coverage.

 

 

What you will do: 

·        Engage in ongoing participant assessment and monitoring and report on services provided and the self-care abilities of participant. 

·        Provide personal care assistance with activities of daily living, including toileting, showers, eating, and grooming.  

·        Assist in the maintenance of physical activity, exercise, and walking. 

·        Identification of safety hazards and emergency situations and initiating corrective action as able.  

·        Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.  

·        Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor 

·        Escort participants to appointments as necessary. 

·        Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PO vehicles as needed. 

 

 

Minimum Qualifications: 

·        High School Diploma or equivalent required. 

·        Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. 

·        Experience in a long-term care facility or with a community-based geriatric program preferred. 

·        Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change. 

·        Ability to work with minimal supervision and exercise independent judgment.  

·        Superior written and verbal communication and presentation skills appropriate for audience comprehension. 

·        Valid drivers license and use of personal vehicle for home care visits 

·        Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service.  

·        Position may require 10% travel within the Centers geographic region and use of personal vehicle.
