A Peer Navigator contributes to AHF by providing peer-to-peer experience in navigating care beginning with linkage to care through Ryan White enrollment and case management. Utilizing their own health experience and a strong skill set in understanding and working through the healthcare system. As a Peer Navigator, you will work with newly diagnosed clients, clients lost to care, and clients currently in care providing them with your experience in the healthcare system. Your compassion and ability to relate to clients provide a level of comfort and trust to our clients. Combined with a thorough understanding of the Ryan White Care Act Program within their specific area, the Peer Navigator collaborates with their Case Management team, Healthcare Center Staff, and Providers so that every Ryan White client receives the support they require and access to available resources.


A Peer Navigator enhances the reputation of AHF by utilizing your own healthcare experience, you will meet our clients where they are, identify strengths and opportunities for growth, and assist them in reaching their goals. You will collaborate with healthcare providers to address any adherence barriers or challenges. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. Your contacts with patients directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

To be successful as a Peer Navigator it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees.
AHF has a collaborative organizational structure where staff are accountable to multiple leaders.


The Peer Navigator will work as a part of a close-knit team to include providing education, emotional support, health literacy, and guidance to ensure optimal adherence to their treatment plans. Working directly within the Healthcare Center, the Peer Navigator is available to talk with every client as needed for better health outcomes and address any adherence barriers or challenges. The PN will serve as a peer mentor to establish a supportive community among clients/people living with HIV.


Licenses and Certifications:

- High School Diploma or General Education degree (GED) is required

- Personal History and experience with Ryan White services

- Compassion, empathy, and the ability to build rapport with patients

- Knowledge of HIV treatment and care guidelines
