Patient Safety Coordinator, Quality

Position Summary: The Patient Safety Coordinator is responsible for coordinating patient safety program activities. The Coordinator assists the Patient Safety Program Manager with the root cause analysis (RCA) process and related actions as requested and has primary responsibility for supporting the National Database for Nursing Quality Indicators program. Additional responsibilities include: assisting with the management of the databases for reporting required core measures, State and CMS measures and assisting the department with preparation and participate in all regulatory surveys including; Joint Commission, State Licensing, CMS etc. Essential Duties:Supervision: + Reports directly to the Patient Safety Program Manager. Supports the National Database of Nursing Quality Indicators program + Coordinates data collection and submission to National Database of Nursing Quality Indicators (NDNQI) for Central Maine Healthcare. + Analyzes data from NDNQI and provides reports as requested by the nursing division and the Director of Quality Services. Coordinates Patient Safety Program Activities + Assists the Patient Safety Program Manager with the Root Cause Analysis (RCA) process including both the RCA meeting and tracking of the follow up actions to completion. + Coordinates the patient safety rounding program: + Produces and distributes a rounding schedule for senior leaders. + Rounds with senior leaders as requested. + Compiles data from patient safety rounds, i.e. safety concerns identified; prioritization of each concern; and next steps/action plan. + Coordinates other patient safety program activities as they are identified. Supports Internal PI Projects; Data management: + Provides clinical data analysis support to CMHC departments, committees and medical staff on key quality and patient safety initiatives. + Compiles, analyzes and reviews data for quality reports. Creates various quality reports for distribution within the organization. + Formats PI data in charts, graphs with accompanying analysis, summary and findings for presentation at committee and Board meetings. + Able to review and analyze internal data reports, external reports and other sources. + Consults other departments as appropriate to collaborate in patient care and performance improvement activities. Facilitates needed changes as directed. Departmental Performance Improvement goals and responsibilities: + Provides back-up assistance for core measure chart abstraction. + Collects patient data through review of the electronic medical record or other departmental records. + Reviews data for completeness and clinical relevance prior to publishing final reports. + Assists with preparation for and participates in all regulatory surveys; Joint Commission, State Licensing Review, HCFA (CMS) Validation Surveys, etc. + Demonstrates knowledge and competence in database use; creates documents, enters data and generates reports in Microsoft excel and MIDAS. + Demonstrates knowledge and competence in data management via electronic environment, including charts, graphs, grids and other accepted forms of data collection, analysis and display. + Communicates appropriately to physicians, staff, Director and administrative team. + Works collaboratively within the Quality department assuming additional responsibilities as requested by direct supervisor or Director of Quality Services to support the needs of the department and the organization.
