Patient Retention Specialist

Position Purpose
As a Patient Retention Specialist at Mercy Care, you will serve as a key connector between vulnerable patients and the comprehensive care they need. Your mission is to help individuals at risk for or living with HIV, Hepatitis C (HCV), and/or syphilis access timely medical care, stay engaged, and thrive through ongoing support and advocacy. You will provide personalized assistance, address barriers to care, and help clients navigate prevention and treatment resources.
 

What You Will Do

Conduct point-of-care STI screenings and assess clients for HIV, HCV, and syphilis risk or diagnosis.

Act as a liaison between patients, Mercy Care’s Early Intervention Clinic, and external healthcare providers.

Link patients to HIV primary care, PrEP treatment, and/or HCV and syphilis care.

Enroll eligible clients in the Ryan White Program and assist with documentation for PrEP Patient Assistance Programs.

Coordinate with clinical staff and medication specialists to schedule appointments and reduce barriers to engagement.

Maintain EPIC referral inboxes and manage a monthly caseload of up to 60 clients using ARTAS 30-60-90 engagement tools.

Conduct regular follow-ups through text, calls, and outreach to ensure appointment adherence and care continuity.

Participate in health fairs, educational events, and community outreach to promote testing and retention in care.

Provide culturally competent support to a diverse patient population, primarily adults but including all age groups as needed.
 

Minimum Qualifications

High school diploma or GED required; associate or bachelor’s degree preferred.

Minimum two years’ experience working with high-risk communities (e.g., HIV counseling, STI prevention, targeted testing, linkage to care).

Experience working in communities at high risk for HIV and/or HCV.  

Valid Georgia driver’s license and ability to travel locally.

Strong communication and interpersonal skills; ability to work independently in a fast-paced, community-based setting.

Proficient in Microsoft Office; familiarity with EPIC and confidentiality in public health.

Strong attention to detail and the ability to manage multiple projects, establish workload priorities, and meet deadlines are critical.  

Knowledge of motivational interviewing, high-impact prevention strategies, public health program implementation and evaluation.
