Job Overview:
The patient navigator collaborates with the women's center providers, all members of the health care team, and community organizations to accept referrals and manage geriatric caseload. She/he works with other healthcare professionals to establish and maintain a climate of mutual respect, dignity, diversity, ethical integrity, and trust. As part of the Women's Center, the Navigator supports and executes the interventions and strategies in a particular territory to impact coordination of care, integration with primary care and other specialty care, patient-centered communication and engagement in care and psychosocial and other supportive care with the goal to improve access to high-quality care. The Navigator supports care that is culturally sensitive and linguistically appropriate, building collaborations with trusted community-based partner organizations that can help address the social determinants of health and the innovative use of digital tools to reduce barriers to care. Functions as medically knowledgeable and friendly resource for patients and caregivers, referring providers, RWJBH providers and staff. Ensures all required demographic, financial, referral/authorization, clinical, and other registration data is accurately scheduled, collected, verified, and communicated appropriately. Functions include scheduling of physician pediatric services for all RWJBH sites, pre-registration/registration, insurance verification, referral coordination, pre-certification, screening for financial counseling, eligibility determination and pre-service communication. Obtains and verifies insurance/payment source, seeks necessary referral/authorization and coverage extensions as required, and assists with identification/resolution of insurance issues.
Qualifications:
Required:
High School Diploma or Equivalent1-3 years of related experience within the healthcare field, required. Ability and interest to learn clinical information and criteria. Knowledge of medical terminology. Emphasis on computer literacy and communication skills, such as Microsoft Excel and Word. Problem solver with entrepreneurial spirit.Preferred:
