GENERAL RESPONSIBILITIES \n
This role works closely with outreach, medical providers, and primary care teams to provide short-term care coordination to improve their health and general well-being through the coordination of care and services available at Terry Reilly under the supervision of the outreach dept. This position works to improve quality, increase patient engagement by helping the patients address the barriers to access care to keep patient active in their care. This position requires the ability to investigate and resolve discrepancies, think critically and analytically, and communicate effectively.\n
MINIMUM QUALIFICATIONS\n\nHigh School Education or equivalent\nOne-year experience in a primary care setting \nStrong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.\nProven record of customer service excellence including good verbal and written communication skills both in person and virtually. \nAbility to navigate multiple computer software programs including EHR, Microsoft Office products, scanner, calculator, and multi-line telephone.\nAvailable for regular weekend and evening duties and multiple non-traditional work sites such as outreach events and home visits.\nAbility to work independently with minimal supervision and use sound judgment.\nWell-organized, creative, flexible, open-minded and able to manage several tasks simultaneously.\nAbility to handle in a professional manner demanding or difficult patients and clients.\n\n
PREFERRED QUALIFICATIONS\n\nPrevious customer service experience.\nPrevious experience in a Healthcare or social service setting with at risk populations.\nPrevious experience with both billing\/scheduling and EHR systems including EPIC.\nBilingual abilities (English\/Spanish).\nKnowledge and experience with health promotion.\n\n
