Responsibilities
Position Summary:
Under the supervision of the operational and clinical managers the patient navigator will support the patient and their family in executing the acute discharge or ambulatory plan of care. The navigator will support continuity by collaborating with the Transitional Care Team by obtaining PCP appointments coordinating transportation addressing needs related to social determinants and support related processes administratively while maximizing the use of technology for efficiency. As a patient advocate it is expected that the patient navigator will build working relationships with the hospital staff PCPs homeless shelters other community partners and connect patients to resources that support positive patient outcomes while empowering them to navigate the health care system successfully. Candidate should have excellent technology skills and be able to effectively work in many software application programs such as Cerner Octavia Word Excel and Netsmart.
Qualifications
Minimum Qualificaitons:
At least 1 year in a health care setting or equivalent education and/or experience in a customer service role with demonstrated verbal and written communication skills The ability to connect with diverse communitiesPreferred Qualifications:
At least 2 years in a health care setting with accountability for working with patients in nursing and/or case management/care coordinationAbility to act with empathy and communicate and follow up with the patient/family for continuity of careAbility to work collaboratively with all care professionals including physicians, nursing, and community based organizations and post-acute care facilitiesCandidates should have excellent technology skills and be able to effectively work in various software application programs.
