The Patient Care Coordinator, in collaboration with, and under the direction of Nursing Leadership participates in the coordination/implementation of fiscal, human and other resources needed to provide patient care services. S/He is a professional registered nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Utilizing the nursing process, the Oncology Nurse Navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The Patient Care Coordinator is a crucial member of the care team and heavily relied on by patients/families, referring providers/ staff, and oncology team to support and guide patients with accurate timely information and communication.
Job Responsibilities
Provides professional comprehensive direct patient care to assigned patients
Coordinates an interdisciplinary plan of care, promoting timely follow up on treatment and supportive care recommendations
Communicate with and systematically assesses the health care of patients/ families and responses to care rendered
Orients patient/family to environment assuring comprehension of information presented
Provides and reinforces education to patients, families, and caregivers about diagnosis, treatment options, side effect management, and post-treatment care and survivorship
Reviews all protocols to maintain quality control and patient safety, and to assist Physicians with patient’s compliance.
Participates in quality meetings to discuss cases and review clinical outcomes.
Meets the educational needs of families and other health care professionals concerning the care of the patients.
Requirements/ Qualifications
Bachelor's Degree in Nursing Required
5 years clinical experience, two of which must be in the specialty or related service; or combination of education and experience, acceptable to Clinical Director of Nursing
NYS Licensed Professional Nurse
Current registration or valid permit to practice in NY state as a Registered Nurse
Certification in specialty highly preferred
