The PAS Resource Specialist in Care Management is a vital member of the interdisciplinary team. This role involves assisting and supporting the Care Management team by providing patients and their families with necessary forms that explain their rights as patients. Additionally, the PAS Resource Specialist aids the Care Management and Social Work teams by assisting with discharge tasks and maintaining discharge resources. This position operates under the direct supervision of the Administrative Supervisor and requires face-to-face interaction with patients and their families in hospital rooms. The PAS Resource Specialist serves as a resource for Important Medicare Messages (IMM), CC44, and discharge appeal processes, ensuring compliance with all regulations.

Function/Duties of Position

Hospital Discharge

Educate patients about the Medicare Important Message process & discharge appeals.  Educate the Care Management teams on the Medicare Important Message processes and update teams on the outcomes/next steps.  Deliver to patients and obtain a signature on the second Medicare Important Message. This includes printing copies for patients if desired and scanning copies into patients' charts via Epic Scanner or using tablets to obtain electronic signatures. Ensure compliance by connecting with all Medicare/MedAdvantage inpatients.  Deliver to patients and scan all Detailed Notice of Discharge forms for Medicare discharge appeals. Includes making a copy for pt after signed. Data entry for UR Committee Case determinations. Create packets under the direction of the Case Manager or CIS  Schedule discharge transportation under the direction of the Case Manager or Care Integration Specialist (CIS)  Performs other tasks as assigned

Administrative Support 

Order and restocking supplies and resources for various Care Management programsTriage incoming faxes and process them appropriately by scanning and uploading documents or distributing them to others.E-mail SNF and home-health availability information by forwarding any e-mailed information and transcribing any faxed information to e- mail.Reconcile P-Card monthlyPerforms other tasks as assigned

Admission and Continued Stay

Deliver, explain, and obtain signatures on Observation notifications to patients as required by the Centers for Medicare and Medicaid Services (CMS). This may include printing a copy for the patient if desired and scanning the copy into the patient's chart via Epic Scanner or using a tablet to obtain an electronic signature.Deliver, explain, and obtain signatures on Condition Code 44 notices. This includes printing a copy for the patient if desired and scanning the copy into the patient's chart via Epic Scanner or using a tablet to obtain an electronic signature.Performs other tasks as assigned Required Qualifications Six months of work experience in a medical office setting, including high volume direct patient contact OR One year of work experience in a high volume direct public contact position Note: Successful completion of a formal OHSU Administrative Internship Program will substitute for one year of experience.

Knowledge and Skills Required:

Basic computer keyboarding skills including typing of 30 - 45 wpm.   Preferred Qualifications Inpatient hospital experience Insurance knowledgeEPIC software. Knowledgeable of medical terminology and patient discharge process. Conflict resolution Additional Details General open office work conditions with frequent interruptions.Meeting with patients and/or families in their hospital rooms, sometimes in cramped quarters, to obtainsignatures on hospital forms.Interacting with nursing unit staff and Care Management department staff.This position requires staff to be able to commute onsite a minimum of 60% of the time or more. (AM for IMsStaff may be sitting at their desk up to 70% of the timeStaff may be required to walk around the hospital up to 30% of the time This position may be located on patient units and requires direct patient interaction in their room. This position may be exposed to contact precautions Requires prioritization. Frequent interruptions in an environment of frequent change and fluctuations. For inpatient units accepts accountability and responsibility. Subject to computer work for several hours, as needed. Able to utilize office equipment within work area.
