A Outreach Worker contributes to AHF by re-engaging clients formerly lost to care, coordinating and implementing specific interventions to access clients who have fallen out of care, and assisting them to return to care. The Outreach Worker (OW) will utilize strong experience in understanding the locality and the specific challenges facing this population and working to find creative solutions to locate and re-engage these clients. By combining their knowledge of the Ryan White Care Act Program within their area, the OW will facilitate enrollment in the Case Management program when clients are re-engaged by working closely with the Specialty Medical Care Manager (SMCM). The OW must attend the monthly Outreach Workforce Committee Meetings conducted by the Ryan White Program. The OW will locate and contact clients compassionately to ensure successful linkage and retention.


A Outreach Worker enhances the reputation of AHF by working to find and re-engage clients and seamlessly transition them to the SMCM with the ultimate goal of viral suppression and retention in care. The Outreach Specialist and SMCM will work as partners in re-engagement efforts. Your contact with clients directly contributes to AHF’s success in meeting the deliverables of our Ryan White/National HIV/AIDS Strategy (NHAS) Supportive contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.


The Outreach Worker will work as a part of a close-knit team to review and investigate the prior client history, implement a plan of action, make phone calls, and visit community sites to locate clients. Re-engaging clients and retaining them in care are the goals of the OW so that viral suppression is achieved, resulting in a healthier population. You will have an opportunity to be an advocate, enhance the reputation of AHF, and encourage clients to continue to seek treatment from an AHF Healthcare Center.


Licenses and Certifications:
- High School Diploma or General Education Degree (GED) required
- Valid Driver License as position will require driving duties
- Able to travel within the servivce area 40% or more each week
- Proficient Outreach Skills
- Knowledgeable about HIV/AIDS and current community resources
- Knowledge of HIV risk behaviors
- Experience in conducting outreach to engage individuals in HIV care
