Summary
Provides professional patient care by evaluating, planning, implementing, and documenting physical therapy treatment. Delivers therapy in accordance with physician orders, utilizing a variety of therapeutic techniques and assistive devices to enhance functional outcomes. Develops individualized treatment plans with measurable goals and updates them as needed throughout the care process. Collaborates with and supervises Physical Therapist Assistants (PTAs) in the execution of treatment plans. Educates patients, families, and caregivers on home exercise programs to support continued progress post-discharge. Ensures timely and accurate completion of documentation and billing. All care is delivered in alignment with the hospital’s mission, rehabilitation department standards, Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.
Education
Must be a graduate of an accredited school of Physical Therapy:
+ Bachelor’s degree required if graduated before 1/1/2016
+ Doctorate of Physical Therapy required if graduated after 1/1/2016
+ International candidates must possess at least a Bachelor’s degree in Physical Therapy
+ Doctorate of Physical Therapy preferred
Licensure/Certification
Must hold or be eligible for a Physical Therapist license in the state where services are provided. Licensure must be obtained within 90 days of employment. CPR certification is required within the first 90 days. All certifications and licenses must be maintained in good standing. APTA membership is preferred. If driving a company vehicle, a valid driver's license is required. If using a personal vehicle, proof of insurance is also required.

Experience
Work experience is preferred but not required, aside from clinical experience gained through educational training.
Other Qualifications
Must be able to lift 50 pounds, assist with 100 pounds, and may be required to push up to 300 pounds. Must be flexible and willing to work at various locations as assigned.
